New and Renewal Cases:

The Administration Department prepares, or partially completes, the initial application(s) and all applicable riders, for both new and renewal sold cases, as well as facilitates the   producer appointment(s) with the carrier(s). Following Underwriting approval of the case, including the signed Disclosure Statement, each case is transferred to Administration. A sold case documents package is then sent, electronically or in hard copy, to the TPA, outlining what is necessary for submission of the case, including, but not limited to, the following (documents are due within the time frame indicated, based on the effective date:

·        Applications and riders that are applicable - completed and signed documents are due at NUS within 15 days.

·        Binder check for first monthly premium – payable to NUS is due within 15 days.

·        Current Plan Document and current amendments – due at NUS within 15 days.

·        New Plan Document, if applicable – due at NUS within 30 days.

·        Current Census at effective date – due at NUS within 15 days.

·        Underwriting requirements – due at NUS as specified or within 15 days. 

·        TPA documents and Producer documents – due at NUS within 15 days.

 

TPA Documents:

·        TPA Questionnaire (in the carrier, SIIA or SPBA format) is due within 15 days.

·        TPA State license for applicable state(s) – due at NUS within 15 days.

·        TPA Errors and Omissions proof of coverage – due at NUS within 15 days

·        Fidelity and/or Crime policy proof of coverage – due at NUS within 15 days.

 

Producer Documents:

·        Producer’s license copy for applicable state – due at NUS within 15 days.

·        Appointment and/or producer form for the carrier(s) – due at NUS within 15 days

·        Certificate of good standing from the state may be required in some instances.

 

Plan Changes:

·        Proposed benefit changes/amendments must be submitted to Underwriting at least 15 days PRIOR to the proposed effective date.

·        Underwriting will advise the TPA and Administration upon approval of the plan changes, including any increase or decrease in the specific rates or aggregate factors or terms of the contract.

·        Administration will issue any policy changes as needed.
